Select an assessment module to begin. Each form follows the same clean multi-step layout, so you can add your own questions and fields later without redesigning the experience.

Start with family profile, address, and household composition details.

How To Use These Forms

1. Select a household before opening any form.

2. Complete the four forms in order: Household Information, SEAF, Engineering Assessment, and Inventory. Each form is linked to the same household ID.

3. Enter the CNIC correctly and fill all required fields before submitting.

4. All calculation fields are completed automatically from the values you enter.

5. After submitting the final form, the household will move to Submitted Forms and will no longer appear in the active household list.
